15-minute visit with a Revolution Health clinician to determine whether bioidentical hormone pellet therapy is appropriate for you. $99. If you join the Male Pellet Membership, this fee is credited toward the $200 enrollment fee. If you have Medicare: Do not purchase this product if you have traditional Medicare (Part B), with or without a supplement. Our physicians participate in Medicare. What this purchase is A paid pellet consultation — not the membership, not an insertion, not labs. No obligation to join. At the visit we decide if pellets are a fit. If you join: $150/month after a $200 enrollment fee. Your $99 counts toward enrollment ($101 remains). Insertions included when appropriate. Labs and supplements are separate. What this does not include Pellet Membership dues, labs, insertion, REV Core / Access. No-show 24 hours’ notice to reschedule. One reschedule included.
