The following Person-Centered Assessments and Support Planning is designed to educate healthcare providers (HCP) on the core principles of meaningful assessments, practical support planning, and responsive care that reflects each resident's needs, strengths, preferences, and goals. What you will learn: Identify resident needs, strengths, risks, routines, preferences, and goals. Recognize changes that may require reporting or review. Use assessment findings to guide daily support planning. Support consistent care through documentation and team communication. Once completed, you will receive a certificate of completion to verify you have finished the training. Course Length: 30 minutes; CME: 0.50.
