The Accident and Incident Investigation Safety Program establishes a consistent process for reporting, investigating, documenting, and correcting workplace accidents, incidents, injuries, illnesses, near misses, property damage, vehicle incidents, fires, explosions, environmental releases, and safety or environmental non-conformances. The primary goal of incident investigation is to determine what happened, identify immediate, contributing, and root causes, and implement corrective actions that help prevent recurrence rather than assign blame. The program addresses immediate emergency response, incident reporting and notification, scene control, evidence preservation and collection, witness interviews, Incident Review Teams, root cause analysis, written investigation reports, corrective actions, lessons learned, injury classifications, recordkeeping, and training. Investigations evaluate factors such as equipment, work practices, procedures, employee training, supervision, environmental conditions, PPE, inspections, and applicable safety requirements. Corrective actions may include hazard elimination, engineering and administrative controls, equipment repairs, procedure revisions, training, PPE changes, and other safety improvements. Findings and lessons learned are communicated through safety meetings, toolbox talks, and training to improve workplace safety and reduce future incidents.
