Whether you're working toward improving blood pressure, blood sugar, weight management, or overall wellbeing, you'll receive personalized coaching rooted in the principles of Lifestyle Medicine and Motivational Interviewing, creating a collaborative partnership that helps you identify what matters most to you and develop practical strategies that fit your life. Our Coaching Philosophy Rather than prescribing solutions or creating dependence, we work together to strengthen your ability to make informed decisions, overcome barriers, and build lasting habits. My role is to offer accountability while recognizing that you are the expert on your own life. This coaching service includes: 3-month program Six sessions total | Two, 30-minute virtual calls or video conferencing (or in-person if pre-arranged) appointments monthly Self-monitoring device awareness using HIPAA-compliant devices and platforms. Choose one connected device based on your goals and/or your physician's suggestion: • Blood pressure trends | Blood Pressure Monitor • Glucose tracking | Blood Glucose Monitor* & CGM • Sleep patterns • Weight and recovery trends | Digital Weight Scale • Activity and hydration awareness • Wellness travel (high altitude) and recovery support, stress management, and nervous system support | HRV, Oximeter *Blood glucose/CGM monitoring recommendations should be discussed with your healthcare provider when appropriate. Staying connected, accountable, and empowered between provider visits is self-agency. Because prevention works best when wellness becomes part of everyday life — not just something addressed after symptoms appear. Monthly Flow 1. 20-Minute Welcome & Technology Setup We'll help you integrate the wearable health technology for home, self-monitoring health metrics, connect your device, navigate the app and establish the cadence for future calls. Secure Wellness App Track your health data in real time, monitor trends over time, and securely communicate with your coaching team. 2. Two 30-Minute Coaching Sessions Each Month Personalized coaching focused on intention, behavior change, lifestyle medicine, goal setting, habit formation, and overcoming barriers using evidence-based coaching techniques. Receive curated resources, recipes, and wellness tools to support your journey 3. Monthly Progress Summary Receive a personalized summary highlighting trends, and data to support your ongoing wellness journey and adjustments as your goals evolve.
