Kind reminder: please do not bring dogs to this hike. PERSONAL DECLARATION AND CONSENT FORM With regard to the event titled “kolektif routes” organized by kolektif on 19.04.2026: 1. I acknowledge that I am personally responsible for any and all matters related to the nature walk, mountaineering, sport climbing, and trail hiking activities in which I am participating; that I have been informed about all risks and dangers associated with such sports; that I accept the risks inherent in these activities; that I do not have any illness preventing me from participating in these sports; and that I am not using any medication nor undergoing any medical treatment. 2. I undertake that before, during, and after the activity, I will not violate safety rules; that I will comply with the instructions and directives of the activity supervisor, coach, and/or leader; and that I am responsible for any problems and/or consequences arising from my contrary behavior or personal faults. 3. I undertake that I will not cause harm to nature or wildlife. 4. I declare that I will not hold the Organizer (kolektif) legally or financially liable, either materially or morally, for any injury or disability that may occur before, during, or after the activity. 5. I accept, declare, and undertake that this document is not merely an information form but also an agreement; that it is signed by me freely, without any pressure, and based on the rights granted to me as a legal adult.
