Sustanon 250mg/ml 5ml vial Sustanon is a mixture of four testosterone esters designed to produce a consistent increase in testosterone during a 3 – 4-week period: The total amount of testosterone in each vial is 250 mg. The vehicle is oily and viscous, and the intramuscular injection (1 mL) should be administered slowly. The vehicle contains arachis oil and Sustanon should therefore be avoided or at least given with caution to clients with a history of peanut allergy. Also, caution in those with history of allergy to soy products as there may be a cross reaction with peanut allergy. Sustanon is not available on the PBS, even for patients with pituitary or testicular disease. The demand for testosterone treatment amongst men is increasing. There are several reasons for this including: Better diagnosis and management of disorders of the pituitary and testes that can cause male hypogonadism. An ageing population. Beyond the age of 45 years, 2% of men are fully hypogonadal and at least 10% of men have compensated hypogonadism (low normal testosterone with increased LH and FSH). A strong association between low androgen status and being overweight or obese. The prevalence of weight problems has increased markedly in the past three decades. Now, two thirds of men in developed countries are either overweight or obese. Widespread use of opiate analgesics. This has reached epidemic proportions and men using potent analgesic drugs commonly have low testosterone status. Increased survival from most common forms of cancer. Chemotherapy often leads to compromised testicular function. Increased recognition of the potential benefits of testosterone treatment along with recent published data relating to the safety and efficacy of androgen replacement. An increasing willingness amongst practitioners to prescribe and supervise androgen replacement treatment. Demand from the male population. Testosterone levels decrease from the age of about 30 onwards at a rate of 2% per year typically. This is partly because of a decrease in testosterone production by the testicles. However, levels of sex hormone binding globulin increase progressively with age, meaning that the levels of free (biologically active) testosterone decrease progressively with age. Symptoms of low testosterone Features of low testosterone vary a great deal from person to person. Also, most of the features are non-specific and may be impacted by other aspects of a man’s mental and physical health. Common features of low testosterone in a man include: Low libido and decreased sexual activity Erectile dysfunction (ED) Infertility – low sperm count Fatigue Decreased muscular strength and stamina Poor recovery from exercise Lack of motivation Decreased mental sharpness and executive function Vasomotor symptoms – sweats and hot flushes Not inclined to initiate activities or to socialise Gynaecomastia – enlargement of breast tissues, sometimes tender Decreased secondary sex characteristics – decreased facial hair and high-pitched voice, although these features are often preserved if they have been present previously Other features include: Low bone mineral density (BMD), low trauma fractures or loss of height Altered body composition with low muscle mass and increased fat mass, particularly visceral fat Low haemoglobin and low haematocrit – decreased oxygen carrying capacity of the blood which contributes to fatigue and lack of stamina Contraindications Active prostate cancer or other hormonally-responsive cancer Poorly controlled mental health or psychiatric conditions Recent (within 6 months) major adverse cardiovascular event Purely for performance enhancement beyond what would be expected for age Competitive or professional athletes or sportsmen Side effects Fluid retention Worsening of sleep apnoea Acne Enlarged or tender breasts (gynaecomastia) Decreased sperm count and/or testicular atrophy Worsening of male pattern hair loss Arterial or venous thromboembolism (if polycythaemia is present) Liver damage – but generally only with high doses
