Intralipid® is a sterile intravenous lipid emulsion used as a calorie-dense energy source and a source of essential fatty acids in patients requiring parenteral nutrition when oral or enteral intake is inadequate or not possible. It contains purified soybean oil emulsified with egg phospholipids and glycerol, forming fat particles that closely resemble endogenous chylomicrons, allowing efficient metabolic utilization. Intralipid provides more than twice the caloric density of carbohydrates, enabling delivery of substantial energy in a relatively small fluid volume. It is considered practically isotonic, which supports administration via peripheral veins in appropriate clinical settings. The emulsion is available in 10%, 20%, and 30% strengths, with increasing caloric density and fat content per millilitre. Clinically, Intralipid is used in pre- and post-operative nutritional support, critical illness, trauma, burns, malabsorption syndromes, prolonged unconsciousness, cachexia, renal insufficiency, and for prevention of essential fatty acid deficiency during total parenteral nutrition (TPN). It has no pharmacologic action beyond its nutritive role and is metabolized similarly to dietary fat. Key considerations include monitoring for hypertriglyceridemia, liver function abnormalities, and rare but serious reactions such as fat overload syndrome, particularly in patients with impaired lipid metabolism. It is contraindicated in severe hyperlipidemia, acute shock, severe liver dysfunction, and known hypersensitivity to soy, egg, or peanut proteins.
